All incidents should be reported via this form.
The Emergency services should only be called once the situation has been
assessed by the Duty Manager or a trained first aider.
If it is clearly an emergency, do not wait and call 999 immediately.
When completing an incident report ensure you capture the following
* What happened?
* Where did it happen?
* When did it happen?
* How did it happen?
* Who was involved?
Ensure that just the facts are captured and that the information is
direct, factual information.
